ACCOUNT TERMINATION

This form provides an alternative option for requesting all your account and user data to be deleted permanently.  We recommend you use the in-app functions provided within MY PROFILE to delete accounts and user data.  Once submitted you will receive an email with a confirmation link in order to verify your identity. Please note that once confirmed, these actions are irreversible.  As such you agree your point balance will be lost – Sports Dynamics LLC will donate any available points to the club for the purposes of funding their youth academy, as outlined in our user terms and conditions available within the mobile app.
